animation_offset=\u00bb\u00bb]\n<p style=\"text-align: justify;\">Una vez retirado el proyecto de reforma tributaria, tras cinco d\u00edas de una fuerte movilizaci\u00f3n social que empez\u00f3 en las calles colombianas el pasado 28 de abril, el punto que tom\u00f3 mayor fuerza en el pliego de peticiones del Comit\u00e9 Nacional de Paro es el retiro de otra de las reformas que el Gobierno Nacional ha impulsado en el Congreso de la Rep\u00fablica<a href=\"#_ftn1\" name=\"_ftnref1\">[1]<\/a>. Se trata del Proyecto de Ley 010 de 2020 en el Senado y 425 de 2020 en la C\u00e1mara, mediante el cual \u201cse dictan disposiciones orientadas a garantizar el Derecho Fundamental a la Salud dentro del Sistema General de Seguridad Social, de conformidad con la ley 1751 de 2015, y la sostenibilidad del Sistema de Salud\u201d<a href=\"#_ftn2\" name=\"_ftnref2\">[2]<\/a>. Este proyecto de ley, al igual que la reforma tributaria, ha sido muy pol\u00e9mico, y aunque cont\u00f3 con las mayor\u00edas legislativas para ser aprobado, muchos son los sectores de la sociedad que rechazan su esp\u00edritu. En un contexto de reformas neoliberales en Am\u00e9rica Latina, el sistema de salud colombiano se configur\u00f3 mediante un mercado regulado de aseguramiento en salud, en donde el Estado regula y el mercado (aseguradoras y EPS [Entidades Prestadoras de Salud) se encarga de la mediaci\u00f3n, articulaci\u00f3n e intermediaci\u00f3n entre los recursos p\u00fablicos existentes y la prestaci\u00f3n de los servicios de salud. Debido a que el modelo est\u00e1 basado en el aseguramiento del individuo y tiene una visi\u00f3n centrada en la enfermedad, muy poco le importa a este sistema las condiciones estructurales que atraviesan la vida de las, les y los pacientes (determinantes sociales de la salud). Bajo estas condiciones, quienes se ven mayormente afectadas son las poblaciones rurales de Colombia, las cuales, adem\u00e1s de tener una presencia d\u00e9bil del Estado y una precaria oferta institucional en salud \u2013lo que se traduce en grandes brechas en el acceso\u2013, no cuentan con m\u00ednimos vitales como acceso a agua potable. Sin embargo, esta parece ser una reforma que no genera cambios sustanciales en la estrutura del sistema. No ser\u00e1 posible hablar de la salud como un derecho fundamental mientras el actual sistema de salud siga operando bajo una l\u00f3gica de rentabilidad; no aborde la fragmentaci\u00f3n del aseguramiento que lleva a que se dividan las actividades de promoci\u00f3n y prevenci\u00f3n (lo colectivo) de las actividades de atenci\u00f3n de la enfermedad (lo individual); siga privilegiando una visi\u00f3n hipermedicalizada de la salud y hospitaloc\u00e9ntrica; no aborde los determinantes sociales de la salud, y no ponga el foco en las inequidades en salud a trav\u00e9s de la incorporaci\u00f3n del enfoque de g\u00e9nero y el enfoque interseccional (interelaci\u00f3n entre clase, etnia, edad, g\u00e9nero y territorio).<\/p>\n[\/fusion_text][\/fusion_builder_column][fusion_builder_column type=\u00bb1_1&#8243; animation_offset=\u00bb\u00bb]\n<p style=\"text-align: justify;\">Llama much\u00edsimo la atenci\u00f3n que esta reforma a la salud, al igual que las que se han adelantado en los \u00faltimos treinta a\u00f1os, no tenga un an\u00e1lisis de sus impactos en g\u00e9nero, tanto por las desigualdades en los resultados y la atenci\u00f3n en salud como por la feminizaci\u00f3n del sistema<a href=\"#_ftn6\" name=\"_ftnref6\">[6]<\/a>. En relaci\u00f3n con los resultados y atenci\u00f3n en salud, preocupa el comportamiento regional, por ejemplo, de indicadores como la tasa de mortalidad materna, el embarazo adolescente y el acceso a derechos de salud sexual y reproductiva. Las tasas m\u00e1s altas de mortalidad materna se presentan en Vichada (547,3), Guan\u00eda (323,3), Vaup\u00e9s (147,3), Choc\u00f3 (130,7) y San Andr\u00e9s (127,9), que son territorios habitados, mayoritariamente, por mujeres ind\u00edgenas, afrodescendientes, raizales y palenequeras. Estas cifras sobresalen respecto a la tasa nacional, que es 32<a href=\"#_ftn7\" name=\"_ftnref7\">[7]<\/a>. Esto sugiere la necesidad (re)pensar el sistema de salud en Colombia bajo la mirada del g\u00e9nero y un enfoque intercultural y regional. Otro es el debate sobre la feminizaci\u00f3n del sistema. Avanzar en el debate de pol\u00edtica p\u00fablica sobre la orientaci\u00f3n de un sistema que garantice la salud como un derecho fundamental, implica incorporar la perspectiva de g\u00e9nero. Desde el OEM proponemos dos elementos inciales para la reflexi\u00f3n. Primero, la necesidad de abordar la <em>producci\u00f3n social de la salud<\/em>, esto es, entender \u201cla salud como un objeto construido y transformado cultural, social e hist\u00f3ricamente como producto de la existencia humana\u201d<a href=\"#_ftn9\" name=\"_ftnref9\">[9]<\/a>, el cual est\u00e1 articulado a la realidad subjetiva de un cuerpo que vive la enfermedad. Esto significa situar a las mujeres en sus condiciones materiales, estructurales y simb\u00f3licas, as\u00ed como desligar sus malestares de la enfermedad. Una de las grandes cr\u00edticas que desde el feminismo se le ha hecho a la salud es la alta \u201cmedicalizaci\u00f3n del malestar de las mujeres, causado por los condicionantes de g\u00e9nero, de manera que transforma en enfermedad individual lo que es violencia estructural, y act\u00faa como herramienta de control social, legitimando la desigualdad y la violencia hacia las mujeres\u201d<a href=\"#_ftn10\" name=\"_ftnref10\">[10]<\/a>. Segundo, privilegiar un modelo basado en la <em>integralidad<\/em> en salud que trascienda el aseguramiento individual y profundice acciones de salud colectiva. Si bien ya hemos planteado que el modelo de aseguramiento presenta un problema general para la garant\u00eda del derecho a la salud, para la equidad de g\u00e9nero presenta un problema espec\u00edfico.
